FREE Flu Shots for Employees, Retirees, and Student with accepted insurance - provided by Staying Healthy Medical Services.
Thursday, October 7
- 9 a.m. - 4 p.m.: Administration Bldg (Classroom AD 2.216)
Friday, October 8
- 9 a.m. - 4 p.m.: Administration Bldg (Classroom AD 2.216)
- 8:30 a.m. - 10 a.m.: Center for Vital Longevity (Break Room, 801)
- 11 a.m. - 2 p.m.: Callier Center Dallas (Room B108)
- 3 p.m. - 4:30 p.m.: Center for BrainHealth (CBH Reception Hall, 1.104)
Other Services:
Vitamin B12 Shots (all locations) - $18.00, cash or check
TDaP Vaccine (Callier Center Dallas ONLY) - Must have insurance or pay $70.00 cash or check
Accepted Insurances: Aetna Choice*, Blue Cross Blue Shield PPO, Cigna PPO, United HealthCare Choice based on plan coverage. HMO PLANS ARE NOT ACCEPTABLE.
Aetna Select Plans cannot be accepted for individuals of the following counties: Collin, Dallas, Denton, Ellis, Parker, Rockwall and Tarrant.
Bring a copy of your Insurance Card, Comet Card and Completed Consent Forms. NOTE: UTD covers flu shot cost for employees without insurance. For retirees and students without insurance, the flu shot is $20.
